This looks like the pieces that make up the disc (the data side and the clear glossy layer) were not glued together correctly when the disc was manufactured, or maybe the adhesive was low quality. I've seen this happen to discs on the inside, in the area where the hole is, but it's never reached the data layer.
But since this is happening from the outer edge, then it definitely means something bad is happening.
or
A transparent layer has delaminated and peeled off by itself from the disc surface.
