Hardware is PS3 Slim. CECH-2503B and BGToolset shows a green tick which I believe means it can be modded. I updated the OFW to 4.88 and then followed the instructions from this video

I've done all BGtoolset steps with no issues, 'NOFSM patch loaded and validated'. I've then restarted the console and when I've gone to install the CFW, once it gets to 70% I get the above error message and then it makes me go back to menu. I have tried different USB sticks, different USB ports and a fresh CFW file download (I can only find the Megaupload source though) and checked MD5 as well and have not found any issues.

Any advice?

My first guess would be that you didn't complete the jailbreak process in other words you loaded the patch successfully but you forgot to apply it.
There is an easy way to confirm that theory, open the PS3 Toolset again. After it initializes, if at the top right of the page it says "custom syscalls: no", it means the patch was not applied, on ofw there is no custom syscalls whereas the 4.88 patch adds standard CFW syscalls.
If the patch was not applied, no big deal, open the PS3 Toolset's Flash memory manager tool, load the patch after it gets validated apply it with the dedicated context menu item. When FMM is done applying the patch (a few seconds) you will get a toast message inviting you to reboot & install the 4.88 CFW of your choice from recovery mode.
