Go to package manager > PSN Download Content and it should be there.

I attached the pkgs for installing/reverting debug settings. I'm interested to see how these work myself since I haven't tested it, but it does exactly the same thing as the debug settings installer, it's just simpler.
Remember, before installing, you need to mount flash or you'll get an error. There should be a flash mounter in your HAN Toolbox.
I rebuilded both, filsystem and database from recovery menu, the damned bug is still there (I'm going to upload a video to show you).

The icon that flash is the MGS GZ one I downloaded in background the first time… It's possible that the download went fine, but for whatever reason I can't see the installation icon??
Never happened this before, and I can't find similar experiences on the web.
It's not a huge problem, I can live with that write on the shut-down options.
However I think that the installation file is there somewhere, wasting some HDD space...

If you can't find it, try using the file browser in HAN Toolbox, go to /dev_hdd0/vsh/task, there will always be a 00000001 folder in there, but look for folders with a different number on the end. This will be the culprit, press triangle and unlink the folder, then reboot. Should solve your problem.
About the debug settings stuff, yes, you will need to enable debug packages and mount flash before installing either one of them. I forgot to mention that I haven't edited a script for these for DEX use, so I would wait before using them.

Can be "unlocked" by using the PARAM.SFO update method... no game truly hard-locks resolution settings AFAIK. The way that it works on the PS3 is the game has a native resolution it will always render at, regardless of XMB setting. If you choose a lower/higher resolution than the native one, the game will just up/downscale to it, this is why performance is exactly the same.UPDATE: Almost every game I played so far output at 60hz (since I changed the console region to North America), all games but GTA 4. This game shows the message that it won't boot with my current video settings. I checked the PARAM.SFO from the BD and actually it have NTSC resolutions unchecked. So this game is region locked.
Rockstar games said that the game was region locked only on XBOX 360, but it's the same on PS3 if you boot it with a SD resolution.
I should ask for a refund...

This may cause issues with some games though, like some UI/HUD elements trailing off the edges of the screen. But if there is a version of the game (say, the North American version) that can run that resolution, then it can also run on the EU version.

I guess you could be patching it incorrectly, why don't you upload the SFO from the disc and I'll patch it for you, I don't mind since it takes like 2mins.However the other games PARAM.SFO I controlled so far, have all the resolution boxes checked. GTA4 has the 480i/p boxes unchecked.

From my (little) experience it seems that resolution parameters are always read from the BD PARAM.SFO (indeed the GD PARAM.SFO that's into the HDD doesn't even have the resolution fields activated).
I tried in any way with GTA4. I tried adding fields to the GD one (that came with the 1.08 update), tried editing the one from the BD too. I edited it in many ways, adding the region field too (tried either with 0 and 1), but it's like the game doesn't care about the GD PARAM.SFO, at least not about the resolution output.
Maybe there's some other file that need to be replaced/edited?

I uploaded both PARAM.SFO, the one from the BD (in the folder GTA4-BD) and the one from the update 1.08 that install into the HDD (in the folder GTA4-UPDATE). They are untouched, I just re-extracted them.
