I would recommend clean, unmodified ISOs first. Then, if all else fails, feel free to mess around. But, for the sake of accuracy, lets try to report only unmodified ISOs, unless the fix was required for the game to run.

Can use a cmdline tool called bchunk.exe like so;

Code: 
bchunk [game.bin] [game.cue] [game.iso]


As for Fatal Frame, I'll rerip and test, it's weird though - usually a random crash is just that, random, but this was specific and always in the same place at the same time.
